Hyperparathyroidism and fibrocystic osteitis: what is important not to miss
Abstract
Primary hyperparathyroidism (PHPT), a common endocrine disease characterized by hypercalcemia and elevated or inadequately normal levels of parathyroid hormone, is diagnosed based on biochemical analyses. Even "asymptomatic" patients may have skeletal abnormalities (evident on imaging; e.g., DXA), and subclinical manifestations may include osteoporosis and hypercalciuria, as well as clinically asymptomatic vertebral fractures and nephrolithiasis. The diagnosis of normocalcemic PHPT can be made after the elimination of the secondary causes of hyperparathyroidism; however, data on its natural course and the corresponding criteria and response to surgery are limited. Parathyroidectomy performed by an experienced surgeon is recommended for patients with symptomatic disease or subclinical damage to target organs, since no drug treatment method eliminates hypercalcemia and the skeletal and renal consequences of PHPT.
