Assessment of insulin resistance in hypothyroid patients

Abstract

Background: Thyroid disorders are one of the most prevalent endocrinopathies across the world. Insulin action has been reported in hypothyroid adults.
 
Objectives: To calculate thyroid profile and to calculate the HOMA-IR based on fasting insulin and fasting blood glucose levels in thesse subjects. 

Materials and methods: The study comprise a total of 164 (82 cases and 82 control) subjects who satisfied inclusion and exclusion criteria. The study was conducted in RRMCH, Bengaluru, South India. Ethical clearance was taken from the institute ethics board. Informed consent also taken. Fasting blood sample were collected to analysed thyroid profile, fasting blood glucose and insulin by CLIA technique. HOMA-IR was calculated by HOMA index= fasting serum insulin x fasting blood sugar (mmol/l)/22.5. Anthropometric measurements, according to WHO 2014 were measured and recorded in centimetre. Statistical analysis: The collected data was analyzed by using IBM SPSS version 22.0 for windows. Descriptive statistics, Student’s t-test Pearson’s correlation coefficient . A p value <0.05 will be taken as statistically significant. 

Results: About 91% subjects in cases and 79% in controls were females. The mean TSH levels (14.26±8.9) of cases were high compared to controls and was statistically significant. 22% of cases as compared to 4% of controls had insulin levels ≥24µIU/ml. The mean serum insulin values were higher in cases than in controls and the difference was statistically significant. HOMA-IR values was slightly higher than controls and the difference was statistically significant (p<0.001). 

Conclusions: Our study demonstrates that hypothyroidism is common among females and is associated with mild degree of insulin resistance. Recommendations: Hypothyroid subjects should be closely monitored for insulin resistance as they are predictive of cardiovascular risk.


Biography

I have completed my MBBS at the age of 22 years from RGUHS, India and MD Biochemistry at 28 years of age from the same university. I am working at Regional Institute of Medical Sciences, Imphal, Manipur, India as an Assistant Professor in the Department of Biochemistry with a teaching experience of 7 years after my post-graduation. Currently I have registered 9 professional memberships. I have attended 93 workshops and presented 12 papers in many conferences. I have completed 21 training programs and certificate courses. I got the best paper award in AMBICON 2020 at National PG update CME organized by Department of Biochemistry, Government Medical College & Hospital, Chandigarh, India. I have a total of 18 journal publications. I have been serving as an editorial board member and have reviewed 8 journals.
