Current scenario: Hidden COVID-19 exacerbating hyperglycemia among diabetes mellitus

Abstract

Current Scenario COVID-19 has been forgotten but not gone and has been present in disguised forms. Now, RT-PCR/RAT has been abandoned. COVID-19 now recognized as multisystem involvement with a broad spectrum of manifestations in different co-morbidities. Therefore, elevated acute blood markers (i.e. any one of the following criteria is 85% and any two are 95%) CRP, Serum LDH, Ferritin, D-dimer level are surrogate tests in view of RT-PCR test with X-ray Chest abnormality, fulfills 100% specificity and sensitivity for the diagnosis of clinical COVID-19. 

Recent evidence had shown that COVID-19 and Diabetes: a bidirectional relationship, that SARS-COV-2 is capable of causing direct damage to the pancreas that could worsen hyperglycemia and can induce the onset of diabetes in previously non-diabetic subjects and/or exacerbate hyperglycemia due to increased insulin resistance and increased oxidative stress, with variable presentations, requiring hospitalization. They were treated with high dose Inj. Artesunate (IV Bolus) for 3 to 5 days, it has pleotropic effects i.e. antiviral, anti-inflammatory, anti-microbial, anti-cytokine, anti-oxidant, immunomodulatory multiorgan protective effects. This is a noble innovative clinical observation and treatment warrants confirmation, also appears to very effective, excellent safety profile, prevents disease progression and its complications, decreasing the morbidity and mortality among the clinical diagnosed COVID-19. For treatment of diabetes, OAD, primarily Dapagliflozin, Linagliptin, Metformin, Pioglitazone along with (if necessary) low dose Insulin was advised for control of blood sugar and excellent hyperglycemic control was observed.

Dr. Gouri Oram has completed her MBBS in the year 1997 & M.D. (General Medicine) in the year 2003 from VSS Medical College & Hospital Burla, Odisha, INDIA.  She joined as Assistant Professor in the year 2003 at the department of General Medicine, VSS Medical College & Hospital, Burla. Now she is Associate Professor in the Department of General Medicine at VSS Institute of Medical Sciences & Research (VSSIMSAR), Burla (formerly VSS Medical College & Hospital, Burla) Odisha, INDIA. She has chaired 3 times in the State Conferences. More than 14 publications in her name in  several reputed journals.
