Targeted therapy in thyroid cancer

Abstract

Precision medicine has transformed thyroid cancer (TC) management by tailoring therapeutic plans to tumor-specific genetic profiles identified via next-generation sequencing. For radioiodine-refractory differentiated thyroid cancer (RAIR-DTC), anaplastic thyroid cancer (ATC), and medullary thyroid cancer (MTC), actionable alterations—such as BRAF V600E, RET fusions/mutations, and NTRK gene fusions—dictate targeted interventions. Selective agents, including multi-kinase inhibitors (lenvatinib, sorafenib) and RET inhibitors (selpercatinib, pralsetinib), alongside redifferentiation strategies, yield significant improvements in progression-free survival (PFS) and overall survival (OS).

However, monotherapies are rarely curative, frequently limited by transient clinical responses and acquired drug resistance. To enhance efficacy and durability, immunotherapy—particularly immune checkpoint inhibitors (ICIs)—has emerged as a vital pillar in aggressive subtypes like ATC and MTC. Combined approaches integrating tyrosine kinase or MAPK/MEK inhibitors with ICIs or TSHR-directed CAR T-cell therapies demonstrate superior antitumor response and counteract microenvironmental resistance, such as TNF-α/TGF-β/LOX signaling pathways.

Despite these clinical advances, critical challenges remain, including toxicity management, drug resistance, limited evidence during pregnancy, and global disparities in drug availability and cost. Emerging technologies—including artificial intelligence for predictive response modeling, biomarker discovery, and CRISPR gene-editing—offer actionable paths toward personalized, durable disease control. Integrating multi-modal targeted regimens with microenvironment-targeted immunotherapies defines the future paradigm for advanced thyroid malignancy management.

Upander Kumar is an Assistant Professor of Surgery at AIIMS-CAPFIMS, specializing in Breast and Endocrine Surgery. Holding an MCh. in the specialty, he is an accomplished academic surgeon with an h-index of 28.
