Efficacy of Vitamin E Versus Pioglitazone in Treating Non-Alcoholic Fatty Liver Disease (NAFLD) without Diabetes
Abstract
Objective: To compare the effectiveness of Vitamin E with Pioglitazone in the management of non-alcoholic fatty liver disease (NAFLD) in nondiabetic patients.
Methods: Ethical approval was obtained from the Ethics Review Committee of Faisalabad Medical University. Patients meeting the inclusion criteria were enrolled after informed consent. Participants were randomly assigned to two equal groups using a computer-generated random number table. Group A received Vitamin E 800 IU daily, while Group B received Pioglitazone 30 mg daily. Patients were followed for three months to assess treatment efficacy by monitoring alanine aminotransferase (ALT) levels according to the operational definition. Follow-up was ensured through telephone contact.
Results: A total of 722 patients were included, with 361 participants in each group. The mean age was 41.25±6.41 years in Group A and 41.23±6.56 years in Group B. Males constituted 60.11% of Group A and 59.56% of Group B. Treatment efficacy was observed in 49.72% of patients receiving Vitamin E compared with 31.30% in the Pioglitazone group (p=0.008).
Conclusion: Vitamin E demonstrated significantly greater efficacy than Pioglitazone in the treatment of NAFLD in non-diabetic patients.

Dr. Ghulam Abbas Tahir is a dedicated medical specialist and Assistant Professor of Medicine with significant experience in clinical practice, medical education, and academic research. He completed his MBBS from Quaid-e-Azam Medical College, Bahawalpur, in 2008 and later earned FCPS-II in Internal Medicine from the College of Physicians and Surgeons Pakistan. To strengthen his academic and teaching skills, he obtained a Certificate in Health Professional Education (CHPE) from CORTEACH in 2024 and also holds certification in Medical Editing and Journalism (CMEJ). Dr. Tahir currently serves as a supervisor for the CPSP Postgraduate Program and the MD Program at the University of Health Sciences, Pakistan, contributing to the training and professional development of future medical professionals. His main academic and clinical interests include internal medicine, diabetes, cardiovascular complications, and evidence-based clinical research. He has authored numerous research articles published in reputable medical journals, addressing important topics such as spontaneous bacterial peritonitis, diabetes management, chronic liver disease, chronic obstructive pulmonary disease (COPD), and cardiovascular outcomes. In addition, he contributed a chapter titled “Emergencies in HIP Diabetes Ketoacidosis in Pregnancy” in the book Diabetes & Pregnancy (2022.
